The rules for proving your DOT medical certification changed in April 2026, at least temporarily. FMCSA granted a nationwide exemption that allows CDL drivers to use a paper medical card as valid proof of qualification. If you drive commercially, this affects you right now.
What the FMCSA Exemption Allows
On April 14, 2026, the Federal Motor Carrier Safety Administration granted a six-month exemption permitting commercial driver’s license holders and commercial learner’s permit holders to present a paper Medical Examiner’s Certificate for up to 60 days after the date it was issued. The exemption runs through approximately October 14, 2026.

Under the 2025 Medical Examiner’s Certification Integration rule, examiners must transmit exam results electronically to FMCSA’s National Registry within one business day. FMCSA then forwards the results to the driver’s state licensing agency, which posts the qualification to the CDL record. In theory, no paper is needed. In practice, the system is not working reliably everywhere yet. Some examiner software fails to transmit. Some state systems lag. Drivers were passing their exams and still showing up as unqualified on their records.
The exemption bridges that gap. Your paper certificate is your proof.
What Drivers Should Do Now
The steps are straightforward, but they matter:
- Keep the paper certificate with you in the vehicle after every exam.
- Confirm within 60 days that your electronic record has posted to your CDL.
- Contact your medical examiner’s office if the transmission has not gone through.
- Plan for the exemption to end in October, when the all-electronic system resumes.
A roadside inspector who sees a paper certificate dated within 60 days should accept it. Enforcement officers have been notified through Commercial Vehicle Safety Alliance channels. If a question comes up, the exemption notice itself is the authority.
Nothing Has Changed About Who Needs the Exam
The exemption changed the paperwork, not the requirement. You still need a current medical certification if you operate in interstate commerce and your vehicle weighs over 10,001 pounds, carries nine or more passengers for compensation, carries sixteen or more passengers regardless of compensation, or transports placardable hazardous materials. The qualification standards under 49 CFR 391.41 covering vision, hearing, blood pressure, and disqualifying conditions remain exactly the same. Certificates are still valid for up to 24 months unless your examiner specifies a shorter period.

Why Timing Matters More Than Usual
Because the electronic transmission system is still unreliable, scheduling your exam early gives you a cushion. A driver who waits until the week their certificate expires has no room for a transmission delay. A driver who recertifies a month ahead can carry the paper card, verify the electronic posting, and never miss a day of work.
This is also worth knowing if you recently moved to Florida. Relocated drivers are one of the groups the exemption specifically addresses, because a new state may not have linked your medical record yet. Your paper certificate covers you while that gets sorted out.
